Hospice Outreach Navigator
We are a faith-based, non-profit home hospice organization with a deep understanding of the unique needs of patients and families needing compassionate end-of-life support. We are seeking a passionate, driven, results-oriented Full-time Hospice Outreach Navigator to join our growing team.
About Us:
With a history of serving our Colorado community for over 100 years, we provide Christ-centered, compassionate hospice services to patients and families in Adams County, Arapahoe County, Denver County, Douglas County, and Jefferson County, surrounding Denver, CO. Our mission is to provide home hospice care, support, and education that honors the dignity of each person we serve.
Outreach Navigator Role:
- Develop and maintain relationships with key referral sources, including skilled nursing facilities, assisted living facilities, hospitals, and local physician networks.
- Develop and maintain relationships with parishes, building long-lasting connections with the parish leadership team and with ministries within each parish.
- Execute a dedicated weekly routing plan to maximize outreach.
- Bring education to both healthcare professionals and individuals on the our benefits of services for patients and families in need.
- Collaborate closely with our leadership and fellow team members to achieve monthly/annual goals.
Ideal Candidate:
- Passionate about providing comfort and support to the lives of our patients and their families.
- Exceptional communication and interpersonal skills.
- 3 + years experience in hospice or home health sales (preferred).
- Open to daily travel and some weekend presentations.
